Questionnaire and Informed Consent Agreement
The MBSR Protocol requires that participants attend the Introductory Session (also known as session 0) or an individual interview with the Instructor before officially registering to the programme. This is to make sure that each participant accepts to fully commit to the programme to the best of their ability, understands that each participant is expected to attend each of the eight weekly session and the full day retreat (for face-to-face programme) or half day retreat (for online programme) and to practise the home assignments for about 45 mins per day during the duration of the training programme.
To register individuals need to be aware of the contraindications, that they do not suffer from acute depression treated with medication and/or active severe mental illness unless approved by a treating medical physician. Each participant is also aware that if for any reason they are unable to, or think it unwise to engage in the gentle stretching practices (yoga) during the weekly sessions or the practices at home, that they are under no obligation to engage in these techniques nor will they hold the MBSR Instructor liable for any injury incurred from these practices. It is important also to note that the MBSR programme is not intended to replace medical or psychological treatment.
Payments are made by electronic transfer to our euro account, details of which will be communicated to you once your application is accepted. Payment must be received once your application has been accepted. No refund will be provided in case of cancellation.

Non-Disclosure & Confidentiality:
a) Each Party agrees to hold any “Confidential Information” (which shall include, but not be limited to, (i) any information (in any form) that is provided by the other Party and is expressed to be or could reasonably be expected to be regarded as being confidential and (ii) any information (in any form) that is derived from any of the information referred to in (i)) in strict confidence, and agrees not to disclose any Confidential Information to any third parties.
b) Each Party agrees to exercise at least the same care in protecting the Confidential Information from disclosure as it uses with regard to its own Confidential Information, which it warrants as providing sufficient and adequate protection from unauthorised disclosure or loss.
